The vessel will be considered for berthing only after submitting the pre-arrival to Port.

• ISPS Form (New)
• Last port Clearance/Last Port Immigration clearance copy (Next Port should be “Fujairah”)
• Certificate Check List
• Fujairah Immigration Crew List – On attached Format – Pls send both PDF & Word/Editable format
• FAL Forms (1 to 7) – Attached – Pls. fill up the all FAL Forms (01- 07) as it is Fujairah Port will not accept any other format of FAL Forms
• NTM Compliance of 148v5 & 225
• NTM Compliance of 271 & 274
• Maritime Declaration Of health from (MDH Annex 8)
• Master Declaration for No arms/Ammunitions

• Mina-Saqr Immigration Crew List – On attached Format – Pls send both PDF & Word/Editable format
• FAL Forms (1 to 7) – Attached – Pls. fill up the all FAL Forms (01- 07) as it is without changing the format and send back each form separately with sign & stamp. Port will not accept any other format of FAL Forms.
